[Bug] Anthropic API Error: 400 Tool Concurrency Issues with Opus Model

Bug Description
When using Claude COde with Opus in VSCode plugin I am getting an error "API Error: 400 due to tool concurrency issues", a few days ago I was getting errors when Opus was thinking and was told that this was due to an incident that happened on the 23rd, now the errors is different but it still seems to be related to Opus and essentially I have not been able to use VSCode plugin with Opus for a while. I will now alos try to see if I encounter the same error with Claude Code command line, but this is getting really annoying.

Notes
The issue does not seem to occur in Claude Code terminal. Thusfar I have only encountered it in the VSCode plugin.
Before the 23rd my VSCode plugin was working really well.
