[BUG] Claude Code doesn't follow CLAUDE.md rules

What's Wrong?

Claude Code doesn't reliably follow CLAUDE.md rules even when they're clearly written. it was clearly written "never guess or aqssume". i asked cloud why were you wrong and the answer was: "I guessed the column names (lesson_name, clip_name) instead of checking the actual table schema first"

What Should Happen?

claude should check what are the column names before creating the sql statement
