[BUG] Messages disappear and neither restarting nor rewinding fix - session becomes permanently unusable

What's Wrong?

Messages appear at the top of the conversation and the same actions appear at the end of the conversation. The LLM thinks it's responding and new follow-up messages are suggested. Rewind and restart don't help. Session becomes permanently unrecoverable. This problem is constant for the past 4 days and so bad that any benefit of Claude Code for practically a whole month could be erased by the time wasted on this.

What Should Happen?

Messages should appear at the end of the conversation so the conversation can continue.
