[Bug] Plan mode exit requires clearing context or auto-accepting changes

Bug Description
Trying to exit plan mode, I now seem to have to choose between clearing contextn (terrible) or autoaccepting (unacceptable).

What is this nonsense? I never want to clear context. It makes Claude BRAINDEAD.
