[BUG] MCP connector shows "connected" but tools become unavailable mid-conversation

What's Wrong?

During a conversation, MCP tools (Linear connector) suddenly become unavailable even though the connector still shows "connected: true" status.

Symptoms:

• search_mcp_registry returns connected: true for Linear
• suggest_connectors shows Linear as connected (no reconnect button available)
• All Linear tool calls fail with "No such tool available"
• Tool ID that worked earlier in the same conversation now fails
• User has no way to reconnect or refresh the MCP session

What Should Happen?

1. MCP tools should remain available throughout the entire conversation session
2. OR if the connection is lost, the connector status should update to show "disconnected" with a reconnect button
3. User should have a way to refresh/reconnect MCP tools without starting a new conversation
