[BUG] After compaction, Claude Code often tries to re-do what it had just been working on

What's Wrong?

After auto-compaction, Claude Code often tries to re-do what it had just been working on (particularly if there had been an official plan file in use), often messing up its own work and/or not realising it had already been correctly completed.

What Should Happen?

The most recent work should be more accurately reflected in the auto-compact context file so Claude Code doesn't confuse what it was working on.
