[Bug] Subagent delegation fails with task ID lookup and API concurrency error on resume

Bug Description
Delegating to user-created subagents fail - first the subagent's Task Output fails with Error: No task found with ID:, and then when main agent attempts to resume the task, the Response is API Error: 400 due to tool use concurrency issues. Run /rewind to recover the conversation.
