Preflight Checklist

  • [x] I have searched existing issues and this hasn't been reported yet
  • [x] This is a single bug report (please file separate reports for different bugs)
  • [x] I am using the latest version of Claude Code

What's Wrong?

When i select lines in VScode editor window and press Cmd + Option + K (MacOS), it doesn't trigger Insert At-Mentioned command even though my Claude Code inline chat opens on the right side bar. It worked yesterday.
I even tried Cmd + Shift + P -> Claude Code: Insert @-Mentioned Reference, it still didn't work. Seems that there's some conflict with the latest VSCode version.
