[BUG] [VSC Extension] Screenshots not getting pasted in the chat context.

What's Wrong?

VS Code "Claude Code for VS Code" extension.
Many times the chat doesn't accept Ctrl + Ved screenshots. Something like this appears:

It was previously working.

What Should Happen?

Claude:

I cannot see images that are referenced by file paths like ''.

To show me an image, you would need to either:

1- Paste/upload the image directly in the chat
2- Give me the full file path and I can read it with the Read tool

Claude himself mentions what should be happening in the extension first.
