What's Wrong?

Sometimes code changes that need approval shows up inline in the VScode extension (windows), other times it opens a window in the editor showing the entire code file that you have to click checkmark on. There is no way to force it one way or another, asking Claude results in it telling me it cant be controlled and depends on size of change, but its untrue as single line changes sometimes open up an entire window in the editor to be approved. Seems once the session is stuck in one way, its always that way. Opening another session will change it sometimes.

What Should Happen?

Show the changes to be approved inline in a box in the extension.

Steps to Reproduce

Don't now how to reproduce, open a new session and ask it to make a change in either plan or permission required mode and cross your fingers.
