[FEATURE] Support submitting plans from CLI to Claude Code for Web

Problem Statement

I find myself using Claude Code for Web less than I would have liked, primarily because I like to get a view of Claude's plans first before fully delegating tasks.

Also, CLI has access to things that aren't currently supported by Claude Code for Web out of the box AFAIK, like MCP servers, Plan Mode, etc.

Proposed Solution

On the plan mode exit prompt, add another option Yes, submit to Claude Code for Web.

Alternative Solutions

Add support for Plan Mode to Claude Code for Web.
But for this to be truly useful, MCP servers would need to be supported too.
