[BUG] Crashes when i press tab to update a command

What's Wrong?

I have a command /ph-workflow:ph-tdd. When i type "/ph-tdd" and press tab to auto-complete the command name, it crashes with below error. tab works fine for other commands/agent names.

What Should Happen?

the full command name should show up

Steps to Reproduce

1. create a local plugin "ph-workflow"
2. add a command "ph-tdd"
3. restart claude with "claude --plugin-dir ~/.claude/plugins/ph-workflow"
4. type "ph-tdd" and press tab to auto-complete. Claude Code will crash at this point
