[BUG] Each write to plan asks "Allow to write to" w/o ability to save choice

What's Wrong?
When I start Plan mode and Clause wants to write a plan in $HOME/.claude/plans it's asking Approve for each edit with Yes/No
What Should Happen?
It should not ask permissions for plan edit

Steps to Reproduce
- I've worked on a Plan A
- Claude done part of the changes in Plan A, but missed some things
- I've started a new session and asked Claude to read Plan A, and create a new Plan B that would be more precise
- Claude asked me about "Do you allow to read plan?", I've selected "Allow for everything" (second option)
- Now every time it tries to write to Plan B and even Plan C, it asks permission for each write to plans

Seems that there's problem with permissions that were added by allowing to read everything in plans folder, so now it has additional permissions check.
Everything is done with Edit automatically
