Steps to Reproduce

Start a session that spawns multiple parallel sub-agents using the Task tool
Let agents run until context gets low
Observe repeated "Context low · Run /compact to compact & continue" messages after each TaskOutput
Try running /compact - doesn't work
Try running /feedback - fails with error

Additional reproduction case

Same issue occurred again - "Context low · Run /compact to compact & continue" appears repeatedly after background task completion:

● Background command "Run Agent R QQQ FRESH (all 422 dates)" completed (exit code 0).
  ⎿  Context low · Run /compact to compact & continue

This time it was a background bash command (not parallel sub-agents), but same behavior:

Multiple tool outputs each showing "Context low" message
/compact doesn't resolve it
Session becomes stuck

The pattern seems to be: any situation where multiple tool results come in sequence triggers this spam.
