[Bug] Claude Code ignores claude.md context file before task execution

Bug Description
I have used and have been using claude code extensively and observe the following issue. Its most of the time do not followthe claude.md files so its good if it can read this type file i mean memory file before every task start. At that time i have seen it performed good. I am a PhD student and using it for diverge task. Another important performance degradation is context compacting. Its lost track completely sometimes and performance degraded. I have made some strick rule in claude.md file but it did not follow that and causes huge data loss in my database. Without my permission it has done it. It means system is dull not intelligent at all.
