[Bug] Repository name/owner lost after auto-compact, invents incorrect GitHub identifiers

Question

**Bug Description**
Claude Code often forgets repo names after compaction and invents the wrong repo owner on github rather than try to figure it out

**Example**

It made up the repo username despite knowing it a few messages earlier.
