[Bug] Terminal setup incompatible detection logic conflicts between Visual Studio and Windows Terminal

Bug Description
Claude code says it cannot run /terminal-setup from 'visualstudio' when I'm actually running it in Windows Terminal. I guess it's just detecting an environment variable that visual studio sets up. So then I tried running it from the Windows default powershell config and now it sais "Terminal setup cannot be run from window-terminal"! Right below is the message telling me I need to run it from Windows Terminal on Windows.
