[Bug] Model selector ignores Claude Opus 4.5 selection, defaults to Opus 4.1

Bug Description
For a long time now, when I select claude opus 4.5 in the model selector, claude code instead selects claude opus 4.1. There is no notification telling me that this has been done, it just happens. It has persisted for over a month now, since the release of the model.
