[Bug] MCP server installation fails silently after restart prompt, requiring manual workarounds

Bug Description
I have trouble asking claude-cli to instal mcp servers, every time it asks me to restart, then the mcp server is not running and i don't know what is happening, and running in circles, only once after very difficult session wit leaving notes in claude.md it managed to install the mcp server but.... it is painfull proces
