[Bug] Claude Code Hangs on Command Output in macOS VS Code Extension

Bug Description
"Claude in VS Code IDE keeps getting stuck while trying to read the output of a command that it ran in the background. I have to interrupt and then launch it in console, then resume conversation to be able to report this feedback since the IDE integrated version doesn't have that option. This only happens on macOS and not my Windows dev machine with WSL or native windows either."
