[FEATURE] Ability to forcibly disable auto-accept

Problem Statement

I never let Claude write code or execute commands without reading it first. I left it with a prompt and came back to find it had somehow got auto-accept mode turned on. I find it highly unlikely I did this intentionally (so whether it was by accident or not, I don't know).

Proposed Solution

Add a feature to Claude's settings file to disable access to the auto-accept mode. When disabled, Shift + Tab would just cycle between plan mode and normal mode.

Alternative Solutions

I don't think there are alternatives. There is no hook for pre-mode change to block it or existing configuration for this as far as I am aware.
