[Bug] Plan mode reuses previous plan instead of generating new plan per session

Bug Description
Plan mode keeps reusing the same plan from previous implementations instead of creating a new dedicated plan for each Plan mode session. For example, if I just request to clean up lint errors while in Plan mode, it will show the previous implementation plan instead of create a plan for cleaning the lint errors.
