[BUG] The @.claude/plans keep resurfacing issues already done.

What's Wrong?

CC writes plans to @.claude\ , then when completing tasks, necros all the finished tasks presenting plans of things already completed. This stuffs the context window breaking the current session

What Should Happen?

Suggestion: either make it work well with proper task tracking or remove it altogether and allow manual md todo list creation instead of defaulting on generated ones.
