[Bug] Plan mode recaps all previous work instead of focusing on incremental changes

Bug Description
Recently Claude has become pedantic about the plan vs edit modes, asking if I can switch modes instead of the previous "allow change, allow changes in session" prompt. And the plan mode is often recapping ALL of the work done in a planning session when I'm just asking for a small change, and I can't work out which part of the plan applies to that new change. The plan is covering all the old work complete. In my workflow, I plan, accept changes, plan another change, etc. It's not one big plan. And I don't want to clear the context because I plan and execute incrementally, towards the same goal. Its quite frustrating.
