[Bug] Claude Code attempts to work on multiple issues simultaneously despite single-issue context

Bug Description
claude code seems to have adopted a new persona which wants to do plan multiple things at once. when it presents is plan it shows actions for multiple issues. i dont feel comfortable seying yes. and i have to reject and ask it not to work on more than one issue at a time
