[BUG] Constant updates to CLAUDE.md

What's Wrong?

This started around 2.0.51.

Previously Claude Code didn't seem to touch CLAUDE.md unless specially instructed to. Now with the latest version Claude Code is constantly updating CLAUDE.md. Claude Code seems to think CLAUDE.md is some developer guidance and updates it all the time.

Is this known issue? Is this something that others are seeing? My team members have reported the same so we've wondering if this is a planned change or not?

What Should Happen?

Should work like previously where CLAUDE.md is not updated unless user asks.

1. Open existing project with CLAUDE.md
2. Do some work
3. Notice how eager Claude Code is to change CLAUDE.md
