[BUG] Pasting new images refer to old images

What's Wrong?

When pasting images and chat with Claude, it replies that the image is an old image it already processed.

What Should Happen?

It should be seeing the new images.

Steps to Reproduce

1. Paste any image
2. Ask it what it sees
3. Exit out of claude
4. Resume the session
5. Paste a new image
6. Ask it what it sees and see it reply with the understanding of the old image.
