[Bug] Plan mode file creation breaks legacy hook execution and ExitPlanMode workflow

Bug Description
plan mode on has changed in an unacceptable ways, it creates a file and it is too disruptive that it stops using ExitPlanMode, we have legacy hooks that make planning well structured and the fact that it saves in $HOME/.claude/plans is just not working, at least, the ExitPlanMode needs to be triggered still in the PreToolUse and PostToolUse
