[Bug] Plan mode incorrectly uses Q&A tool instead of plan execution tool in Opus 4.5

Bug Description
after using opus 4.5, the agent does not properly present plans in plan mode as before. the model will use the plan mode's question/answer tool to ask if the plan is correct. it does not use the correct tool to ask to execute or reject the plan.
