[Bug] Premature context low warning at 20% token usage with Sonnet 4.5 1M

Bug Description
The Sonnet 4.5 1M model says "Context low · Run /compact to compact & continue" while only "claude-sonnet-4-5-20250929[1m] · 195k/1000k tokens (20%)" is used and it won't continue. It's stops writing code during a session and compacting at this point will lose a lot of context.
