[Bug] "Always allow" settings not persisting to .claude/settings.local.json

Bug Description
After some Claude Code update it stopped updating in project .claude/settings.local.json when I say "Always allow ...", so this always allow now only works for a session. If I add something there myself I still get confirmation dialog for commands there sometimes
