[BUG] Claude thinks background processes are running

What's Wrong?
Claude consistently says "I notice you have 11 background processes running (mostly querying promotion/campaign tables). Would you like me to..." or similar when doing /clear often in a long session where I have done many background processes.
What Should Happen?
Claude should know if they are running or not and don't reference them if they are not running.

Steps to Reproduce
- Ask Claude to create a script to do something
- Ask it do run it in the background a few times
- Ask it to write a handover document to continue in a new chat
- /clear
- Ask it to read the handover - it should now reference old background jobs that are finished, asking you if it should check the output for progress or kill them
