[BUG] Slash command search suggestion renders

What's Wrong?

The terminal is Ghostty.

Under certain conditions, the slash command search suggestion list contains duplicate items.

What Should Happen?

Obvious. Sane search suggestion list

Error Messages/Logs

Steps to Reproduce

  1. Add slash commands with names, aaa and bbb.
  2. Start the Claude Code and type /. The result is already glitchy. Duplicate items are shown.
  3. Type a and delete it. There are now two entries of /bbb.
  4. Repeat the step 3 and you get another /bbb entry every time.
