What's Wrong?

The terminal is Ghostty.
Under certain conditions, the slash command search suggestion list contains duplicate items.
What Should Happen?
Obvious. Sane search suggestion list

Steps to Reproduce
- Add slash commands with names,
aaaandbbb. - Start the Claude Code and type
/. The result is already glitchy. Duplicate items are shown. - Type
aand delete it. There are now two entries of/bbb. - Repeat the step 3 and you get another
/bbbentry every time.
