[BUG] Too aggresive file and tool output caching/memory

What's Wrong?
When I use prompts like Do tasks in tasks.md file or Do tasks in TODO comments in modified files as indicated by git status command, Claude Code often ignores the recent changes, despite e.g. running cat tasks.md or git status shows them (i.e. it's the Claude overcaching something, not editor delaying saving).
Restarting claude session doesn't help, only waiting some time seems to work.
What Should Happen?
Files should be re-read, git status should be re-run.
