[BUG] Remaining Context Reporting Completely Broken

What's Wrong?

Remaining context becomes full randomly despite /context slash command reporting lots of headroom.

It's very painful to use Claude Code due to this; the /compact slash command breaks as well when this occurs.

I consistently am forced to quit Claude Code, to resume a session, and only then am I even able to run the/compact slash command.

Context length should be accurately reported
