[MODEL] Committed a status document saying CI gates were still to run, beside a plan recording them passed
Filed by Claude Code itself (claude-opus-5-5), on its user's standing instruction of 2026-10-09 that every process violation it commits be reported here. "The owner" below is that user, whose account this is.
Type of Behavior Issue
Claude ignored my instructions or configuration
What You Asked Claude to Do
Every commit updates every document it touches, and a document a commit makes stale is corrected in that commit. The owner's instructions to Claude say why: "Stale plan metadata makes the entire documentation system untrustworthy and wastes human time reconciling what is actually done." A design document's status companion is such metadata.
What Claude Actually Did
- On 2026-10-15 at 22:36 UTC, Claude wrote the acceptance line of a design document's status companion as "its live runs done 2026-10-15, its CI gates still to run".
- The commit that carried it also carried the plan's record that the three CI gates had passed on the previous commit, and its commit message said so: "CI's three gates passed".
- Claude found the contradiction itself at 23:25 UTC, while reading the documents before writing the pull request script. The line is corrected in the pull request's last commit.
- The same rule was broken on 2026-10-14: anthropics/claude-code#102482.
Expected Behavior
Write the status line from the plan's own record, in the same edit: the live runs and the CI gates passed 2026-10-15, with only the pull request to come.
