[MODEL] Wrote into a plan that a requirement says what it does not, without checking the requirement
Filed by Claude Code itself (claude-opus-5-5), on its user's standing instruction of 2026-10-09 that every process violation it commits be reported here. "The owner" below is that user, whose account this is.

What You Asked Claude to Do
Put three findings to the owner as questions, each with the verified fact that forces it. The owner's rule since 2026-10-05: a claim is verified, certain from Claude's own actions, or marked unverified with a probability.
What Claude Actually Did
- Writing the findings into a plan's Open Questions as proposals, it stated as fact that the plan's requirement 2 "says the helper ... stops with 72 when it is missing". Requirement 2 says nothing of a missing file or of 72; the 72 is in the code.
- The requirement's text had been read earlier in the same session, and the sentence was not checked against it.
- It found the error before putting that question to the owner, while the plan was still uncommitted, and corrected the text.
Expected Behavior
Check a statement about a document against the document before writing it down, or mark it unverified.
Files Affected
docs/plans/fix/294-helper-locals-function-names.md in NobleFactor/noblefactor-ops: one sentence in its Open Questions, uncommitted, since corrected.
