Filed by Claude Code itself (claude-opus-5-5), on its user's standing instruction of 2026-10-09 that every process violation it commits be reported here. "The owner" below is that user, whose account this is.

Type of Behavior Issue

Claude ignored my instructions or configuration

What You Asked Claude to Do

Write the commit script for a phase of planned work and hand the owner the one command that runs it: the owner runs every commit script, and the agent never does. Claude Code's own instructions say that when the user needs to run a shell command themselves, Claude suggests they type ! <command>, which runs it in the session so its output lands in the conversation.

What Claude Actually Did

  1. It wrote the script, checked it, and ended its turn with the command as a bare path in a code block, without the !.
  2. The owner: "where's my bang?" The owner then ran it with the ! themselves.

Expected Behavior

Give the command as ! <path to the script>, ready to paste and run in the session, as Claude Code's instructions say.

Files Affected

None. The command was right; its form was not.

Permission Mode

Accept Edits was ON (auto-accepting changes)

The session runs in Claude Code's auto mode.

Can You Reproduce This?

Sometimes (intermittent)

Steps to Reproduce

Not reduced to steps.

Relevant Conversation

The commit script is /Users/david-noble/Workspace/NobleFactor/go-devlore-cli. It runs make check, commits three files (the plan, install.sh and scripts/Test-InstallScript.sh) and pushes. To run it: `` /Users/david-noble/Workspace/NobleFactor/go-devlore-cli ``

The owner, next: "where's my bang?"

Impact

Low - Minor inconvenience

Anthropic API

Additional Context

Cost: 1,930,864 tokens and $1.00, over 6 calls: 1 that committed the violation and 5 that found, disclosed and reported it (Opus 5.5 list prices, from this session's per-call usage).
