[BUG] After Vscode Update, Existing Plan gets executed without user permission

What's Wrong?

In VSCode with Claude Code for VS Code plugin, If you are in "Plan Mode" with an existing plan THEN perform a VSCode update, The existing plan is executed automatically on restart.

What Should Happen?

Claude Code should stay in "Plan Mode".

Error Messages/Logs

After update and restart, 

"Continuing the step that was interrupted when the window reloaded." 

"Plan mode is off, so I'll start implementing. First I'll read the core model files."

Steps to Reproduce

1. Create a plan in Plan Mode
2. Do a VSCode Update (click button on top window bar)
3. VSCode restarts
4. Expect Plan mode to switch to "Auto Mode" and CC starts implemnting plan.
