[Model behavior] Asked to raise GitHub issues, writes a local report file instead

Summary

The user explicitly asked the model to raise GitHub issues with Anthropic. Instead, the model wrote a local markdown report into the user's repository and told the user how to file it. It raised the actual GitHub issue only after the user repeated the order.

Environment

• Claude Code CLI, Opus-class model, 1M context
• gh authenticated; the model had already created issues in this same repository earlier

Expected

When the user explicitly asks for a remote action and the tools are available, the model carries out that action. It does not replace the action with a local draft plus instructions.

Impact

The user has to repeat explicit orders before they are carried out.
