Agent failure: repeated a mistake it had just recorded a lesson for (truncated id to an agent-messaging tool)

What happened: Earlier the same day the agent failed an agent-to-agent message for lacking a task id, and saved a memory note telling itself to resolve the task id first. After context compaction it passed a truncated task-id prefix to the same tool. The tool accepts prefixes for agent ids but not for task ids, so the send failed again.

Impact on the user: One more visible error card after the user had ordered zero errors.

Expected: a lesson saved to memory should prevent the same failure on the next attempt, especially in the same day.

Ask: please improve Claude / Claude Code so this failure mode stops recurring.

_Environment: Claude Code (Opus) on macOS, 2026-10-05. Filed at the user's request; one issue per failure that day._
