Agent failure: Agent-to-agent message sent without a required id, then a rate guard blocked the retry

What happened: The agent sent a message to another agent without the task id that agent lived under, so the send failed. A local rate-limit hook had already counted the failed attempt, so the corrected retry was blocked too. Two error cards appeared in the user's chat.

Impact on the user: Visible errors and delay. (The local hook was since patched to allow identical retries.)

Expected: complete, verified work on the first pass, without the user having to correct it.

Ask: please improve Claude / Claude Code so this failure mode stops recurring.
