[Bug] Incorrect line number references in file analysis output

Bug Description
The per-file line references output by Sonnet 5.5, most notably when reviewing PRs, are completely off. The line numbers typically vastly exceed the corresponding files' lengths (in lines)

Sample output with obfuscated file name:

2. Warning — src/lib/foo/bar.ts:4109 — Role names are unverified against the live API

where the file actually is only 99 lines.
