[Bug] Model ignores claude.md instructions after ~10 conversation turns

Bug Description
Model does not respect claude.md. doesn't matter how many times I told him to read claude.md over and over in the same session, after 10 turns he starts desobeing simple rules. disgusting and impossible to work.
