[Bug] Claude denies previous statements in conversation history and exhibits inconsistent memory recall

Bug Description
Claude wrote something that was wrong. When confronted about it, it denied ever saying that. When I asked it to check its own session log it noticed "weird output" that it told me it never said and then the safety guards kicked in.
