[BUG] VS Code extension lists desktop-app local sessions but opens them as a read-only snapshot (no continue, no Remote Control)

Steps

1. In the Claude Code desktop app, start a local session (Remote Control enabled).
2. In the VS Code extension, open Past Conversations.
3. Open that session.

Expected

The session opens live and is resumable. Remote Control is shown as available, and the full session detail is visible.

Actual

The session is listed, but opening it shows only a static snapshot of the desktop-app session. There is no continue action and no Remote Control indicator, so the level of detail visible in the desktop app cannot be seen in VS Code.
