[Bug] MCP plugin servers disabled per-project don't persist across no-folder sessions

Bug Description
Plugin MCP servers turned off in /mcp are only disabled per project (stored per project in ~/.claude.json). In sessions with no project folder, such as the desktop app's "No folder" Code sessions, the Vanta plugin's AUS and EU servers load again and show as unauthenticated. Every turn, the app then tells Claude to tell me they need authorization, so unrelated answers end with a sign-in reminder for servers I deliberately turned off. Requests: (1) a global way to turn off one server from a plugin, and (2) don't ask Claude to relay sign-in notices for servers I've turned off in another project.
