[BUG] Panel opened through the vscode://anthropic.claude-code/open link has no title row

What's Wrong?

A Claude panel opened through vscode://anthropic.claude-code/open has no title row: the session name, rename pencil, history button and new-conversation button are all missing. Panels opened from inside VS Code show the title row as usual.

What Should Happen?

The same title row as a panel opened from the extension.

Steps to Reproduce

1. Open vscode://anthropic.claude-code/open, for example with Start-Process in PowerShell.
2. The panel opens without the title row.
