[BUG] /insights generates empty report — facets directory never created (still occurring)

What's Wrong?

Running /insights produces only the quantitative/stats sections (tools, languages, time-of-day, errors); all AI-narrative sections ("What You Wanted", "Impressive Things", "Friction", "Features to Try", etc.) are empty, and the insights payload is {}.

• ~/.claude/usage-data/facets/ is never created
• At-a-glance shows "No insights generated"
• Reproduced on two consecutive runs in the same session (re-running does not help)

Likely duplicate of (all closed without fix): #70011, #81679, #64275, #26705, #24039 — same root cause reported repeatedly since February 2026.

What Should Happen?

/insights should generate a working report with all contents present. Should create ~/.claude/usage-data/facets/.

Steps to Reproduce

• Call /insights
• Open the HTML report
• See the narrative sections are missing and the navigation/tabs at the top do nothing because there's no related sections/contents.
