[feedback] provenance: Values typed instead of read: a filename from a truncated display, a landing sha from git log -1 on a shared t
Context. Solo-operator project using Claude Code (desktop app, Windows 11) with a lead session on Claude Fable 5.1 orchestrating Opus 5.5 / Sonnet subagents (worktree-isolated), a JSON task store, PreToolUse/SubagentStop hooks and a harvest-train commit flow. This finding was measured on or before 2026-09-22 and filed on 2026-09-28 at the operator's request; numbers are from tool output. No secrets, identities or private paths.

Finding. Values typed instead of read: a filename from a truncated display, a landing sha from git log -1 on a shared tree, four board ids from a grep over prose, a record id (#1241) from memory.
Evidence (measured). 13 lead hand-input errors in four hours; 18 wall events on the lead's inputs vs 0 on generated code (ledger 2026-09-22 LESSONS RECORD).
Minimal reproduction. Show the model a tool output with a truncated line, then ask it to act on the identifier; or run two writers on one tree and ask for the sha of my commit.
Ask. Tag emitted identifiers by provenance (read-this-turn vs recalled); refuse to put a recalled identifier into an action.
